We are available to assist you by phone and appointment during regular office hours. After hours, you may leave a message on our answering system.
In addition, we also offer a secure online patient portal where you can:
- request prescription refills
- update your contact and insurance information
- manage your Health Summary (medications, problems, allergies, medical history)
- view and export your Chart Summary
- receive messages and lab results from your provider
If you’d like to create a portal account, please sign a portal authorization form. These forms may be picked up in the office or sent to you upon request. Note: The portal is not accessible outside of the United States.
