Health Coaching

A health coach is your personal health guide, mentor and supporter. They help you make lifestyle changes to improve your physical and emotional health. Your practitioner may suggest working with our health coach, Jordyn Croteau, MS, GGS-1, to support you in reaching certain goals. Our health coaching services are available to all patients whether by practitioner recommendation or patient interest!

Health coaching is designed to help you make meaningful changes related to:

  • Physical activity.
  • Nutrition.
  • Weight management.
  • Stress management.
  • Sleep

If one of these areas is out of balance, the others may be as well. Combined, it might feel like too much to solve. That’s where health coaching can help you to sort out where to start and find ways to make small changes that grow over time.

Jordyn will help you set reasonable goals and develop a plan for reaching them. She may also help you figure out what is keeping you from reaching those goals. For example, maybe you don’t think you’re capable of making the changes your provider is suggesting. 

Health coaching helps you take a look at all the factors that might be affecting your health-related behaviors. And together, you come up with a manageable plan for working toward your goals.

Jordyn can help to keep you accountable in reaching your health goals, checking in with you through secure email, phone, virtually or in-person appointments. During these conversations, she’ll ask you questions to get you thinking about how you can overcome challenges, integrate your care plans from your provider and build lasting behaviors to improve your overall well-being.
