The secure compounded medication refill form is for use by established patients with active prescriptions only. 

 Click on the “Refill Here!” button below to refill your prescribed compounded medication(s) including:

• BHRT (Bio-Identical Hormone Therapy)
• LDN (Low Dose Naltrexone)
• Compounded Injectable Medications 

Please fill out all necessary fields, sign & submit to refill. If there are any issues with your refill a patient coordinator will reach out. 

Please allow 10-14 business days to receive your medication.
